Why it matters

Warm bathing before bed is one of the oldest and cheapest sleep rituals going, and it has picked up a scientific rationale as passive body heating. Individual trials have reported that immersing the body in warm water can help people fall asleep faster or sleep more soundly, but they are small and use different water temperatures, durations and timings relative to bedtime. Before treating a nightly soak as a genuine sleep intervention, it is worth knowing whether the pooled evidence shows effects on measured sleep, on how sleep feels, or on both.

What they did

The authors conducted a systematic review and meta-analysis restricted to randomised controlled trials of water immersion thermotherapy in healthy adults. They searched an unusually broad set of databases, including PubMed, the Cochrane Library, Web of Science, Embase, Scopus, Ovid MEDLINE, Wiley Online Library, PEDro, and the Chinese-language CNKI, SinoMed and Yiigle. Ten randomised controlled trials met the criteria. Outcomes were pooled as standardised mean differences with 95% confidence intervals, using a common-effects or random-effects model as appropriate, and covered total sleep time, sleep efficiency, sleep latency and the Pittsburgh Sleep Quality Index, including each of its component subscales.

What they found

Water immersion thermotherapy significantly increased total sleep time (SMD 0.63, P = 0.0438) and significantly improved sleep efficiency (SMD 0.64, P = 0.0419). Sleep latency did not reach significance (P = 0.0877). Neither did the Pittsburgh Sleep Quality Index total score (P = 0.1136), nor any individual subscale: subjective sleep quality, sleep latency, sleep duration, sleep efficiency, sleep disturbances, use of sleep medication and daytime dysfunction all failed to shift. The authors conclude the sleep-improving effect exists but has clear limitations, given the mismatch between the measured gains and the unchanged self-reported picture.

Where it fits

This supports the general idea that pre-sleep warming can modestly affect sleep architecture, while tempering the more enthusiastic version of the claim that a hot bath transforms how well you sleep. The divergence between objective gains and flat questionnaire scores is the interesting complication: either the objective changes are too small to notice, or the PSQI is not sensitive to them over the short trial windows involved. Open questions include the optimal water temperature and timing before bed, whether effects persist beyond a few nights, and whether people with disturbed sleep — not just healthy adults — would respond differently.

What it means for you

A warm soak before bed is a reason to expect a small increase in how much of the night you actually spend asleep, rather than a fix for feeling unrested. Do not expect it to make you fall asleep noticeably faster, since sleep latency did not change significantly here. Because the trials were in healthy adults, this says little about insomnia or other sleep disorders. Treat it as a low-cost, plausibly helpful habit whose measured benefit is real but modest, and whose felt benefit was not detectable in these trials.
