Remote pelvic floor training pays off postpartum

Why it matters
Urinary incontinence is a common problem that affects women's physical and mental well-being, and pelvic floor muscle training is the mainstay conservative approach. Getting that training reliably, though, usually means clinic appointments, supervision and repeat visits, which many women cannot easily fit around work or a new baby. Online and app-based delivery promises the same exercises without the travel, but it also removes the hands-on coaching that supervised programmes rely on. The open question is whether remote delivery still changes symptoms, and whether it works equally well for a woman weeks after birth and a woman in later life.
What they did
The authors ran a systematic review and meta-analysis of randomised controlled trials, searching nine databases for studies of online pelvic floor muscle training in women with urinary incontinence. They extracted data on incontinence incidence, symptom severity, quality of life and exercise compliance, assessed risk of bias with RoB 2 and rated certainty using GRADE. Random-effects meta-analyses were run in RevMan 5.4, with results separated by life stage so that postpartum women and elderly women could be compared. Sixteen studies met the inclusion criteria.
What they found
Among postpartum women, online training significantly reduced the incidence of urinary incontinence, with a risk ratio of 0.56 (95% CI 0.38 to 0.82), and produced a small reduction in symptom severity (SMD -0.25, 95% CI -0.45 to -0.05). In elderly participants the severity effect was smaller and not statistically significant (SMD -0.12, 95% CI -0.25 to 0.02). Exercise compliance improved in both subgroups, suggesting the remote format helped women keep going. No significant difference emerged for quality of life, and moderate-to-high heterogeneity was observed across the pooled studies.
Where it fits
Supervised pelvic floor muscle training already has a solid evidence base, and this review extends the question to whether the same exercises delivered online retain their effect. The postpartum result is consistent with the idea that motivated younger women can learn and progress the exercises remotely. The weaker showing in older women complicates any blanket claim for digital delivery and raises the possibility that established, longer-standing symptoms need more support than a screen provides. Substantial heterogeneity and the absence of a quality-of-life signal leave open how much of the benefit patients actually feel day to day.
What it means for you
This is a reason to think that remote, guided pelvic floor programmes are a credible option rather than a watered-down substitute, at least in the months after childbirth. The consistent improvement in compliance across ages is arguably the most portable finding: people appear to stick with these programmes when they are delivered digitally. For older women, the evidence here supports better adherence but not a clear reduction in symptom severity, so expectations should be set accordingly. Anyone with persistent symptoms is looking at a question for a clinician, not a review of pooled averages.
