Front-loading protein at breakfast missed on muscle
Pooling seven randomised trials in adults aged 60 and over, adding protein or amino acids at breakfast did not improve lean mass, grip strength or lower-limb strength. The evidence was judged very uncertain, and in trials without exercise the extra morning protein was linked to slightly lower total daily energy intake.
Why it matters
Preventing sarcopenia — the age-related loss of muscle mass and strength — is a major goal in older-adult health. Beyond hitting a daily protein target, a popular idea is that the distribution of protein across the day matters, and that older people typically eat least protein at breakfast. Loading more protein or amino acids into the morning meal, while keeping total daily intake the same, has therefore been proposed as a low-cost way to support muscle. But the optimal timing of protein consumption remains uncertain, which is exactly what nutritional recommendations need pinning down.
What they did
The team ran a systematic review and meta-analysis of randomised controlled trials comparing morning protein or amino acid intake with no such intake in healthy and frail adults aged 60 and over. A trained librarian built search strategies across MEDLINE, CENTRAL, EMBASE and ClinicalTrials.gov. Two reviewers independently screened studies, extracted data and assessed risk of bias with Cochrane's RoB tool 1.0. Standardised mean differences were calculated in Review Manager. Seven RCTs involving 305 participants met the criteria, with outcomes spanning lean mass, grip strength, lower-limb strength and total daily energy intake.
What they found
For lean mass, five studies with 181 participants gave a standardised mean difference of -0.11 (95% CI -0.41 to 0.18) — no benefit. Grip strength across six studies and 274 participants was likewise flat at -0.01 (95% CI -0.25 to 0.23), and lower-limb strength across three studies and 157 participants matched the lean mass figure at -0.11 (95% CI -0.41 to 0.18). The one signal that emerged was on eating: morning protein intake was linked to a marginal reduction in total daily energy intake (SMD -0.35; 95% CI -0.70 to 0.00), which became significant in trials that did not include exercise (SMD -0.47; 95% CI -0.82 to -0.11).

Where it fits
This complicates the widely repeated suggestion that shifting protein towards breakfast is a promising lever for muscle health in later life. The pooled estimates were all close to zero and their confidence intervals spanned both benefit and harm, and the authors explicitly describe the evidence as very uncertain. The hint that extra morning protein slightly displaces total daily energy — particularly without exercise — raises the question of whether front-loading protein could inadvertently suppress appetite in a group already at risk of under-eating. Open questions include whether the trials were long enough, whether frail and healthy older adults respond differently, and whether protein timing only matters alongside resistance training.
What it means for you
If you have heard that a big protein breakfast is the key to protecting ageing muscle, this pooled analysis is a reason to hold that idea loosely. The available trials, small in number and in participants, did not show gains in muscle mass or strength from adding protein in the morning. It also raises the possibility that morning protein nudges total daily intake down when no exercise is involved, which matters for people whose appetite is already limited. Total daily protein and training remain the parts of the picture this review did not test.
