This review set out to gather the recent evidence on pomegranate supplementation in athletes and physically active people across different disciplines.

Men made up the large majority of participants, with 155 males compared with 21 females. Just under half — 45% — of the research described was conducted on athletes, while the rest studied highly active participants. Randomised, crossover, double-blind designs made up the majority of the experimental designs used. Supplementation differed considerably between studies in form (pills or juice), dosage (from 50 ml to 500 ml) and duration of intervention (from 7 days to 2 months).

Across the reviewed articles, pomegranate supplementation was associated with improvements in whole-body strength and in reported feelings of vitality, as well as reductions in acute and delayed muscle fatigue and soreness. Vascular measures improved, with increased vessel diameter and blood flow, alongside a rise in serum total antioxidant capacity. The rate of increase in heart rate, systolic blood pressure and the muscle-damage markers creatine kinase and lactate dehydrogenase was reduced, and post-training markers returned towards baseline faster. Markers of inflammation and oxidative stress fell, and the activity of the antioxidant enzymes glutathione peroxidase and superoxide dismutase rose. The authors reported a positive effect on a variety of parameters in the majority of studies.
Where it fits
This is a narrative synthesis rather than a pooled analysis, so it describes the direction of findings without quantifying how large any effect is or how consistent it is across trials. The heavy male skew — 155 of 176 participants — leaves the picture in women thin, and the wide spread of forms, doses and durations means no single regimen has been tested repeatedly enough to be considered established. It sits alongside a broader literature on tart cherry and beetroot in which promising marker changes have not always translated into performance a coach would notice. Larger, dose-standardised trials with performance outcomes as the primary endpoint are the obvious next step.
What it means for you
The honest reading is that pomegranate has repeatedly shown favourable movement in recovery and blood-flow markers in small studies of active people, which is a reason for interest rather than confidence. Because doses spanned an order of magnitude and forms differed, there is no basis here for saying what amount would matter or in what format. The near-absence of female participants is a genuine limitation for half the readership. Marker improvements are also not the same thing as a better session or a faster race, and those harder outcomes remain lightly tested.
