Why it matters
Physical activity is widely accepted as beneficial for healthy ageing, and there is a plausible case that it also helps maintain cognitive function into older age. What has been far less clear is whether cardiovascular fitness specifically — the kind built through aerobic exercise — is actually necessary for any cognitive benefit, or whether movement of any sort might do. Answering that requires studies that measure activity, fitness and cognition in the same individuals. This Cochrane review set out to assess whether aerobic programmes aimed at improving cardiorespiratory fitness improve cognitive function in older people without known cognitive impairment.
What they did
The reviewers searched MEDLINE, EMBASE, PsycINFO and several other databases up to 15 December 2005, with no language restrictions, for randomised controlled trials in people older than 55. Eligible trials compared aerobic physical activity programmes against any other intervention or no intervention at all. Eleven RCTs met the inclusion criteria, and two reviewers independently extracted the data. Outcomes covered both cardiorespiratory fitness — assessed by the maximum oxygen uptake test, considered the single best indicator of the cardiorespiratory system — and a range of cognitive functions.
What they found
Eight of the 11 studies reported that the aerobic intervention improved cardiorespiratory fitness by approximately 14%, and this improvement coincided with gains in cognitive capacity. The largest cognitive effects appeared for motor function (effect size 1.17) and auditory attention (0.50), with moderate effects for cognitive speed (0.26) and visual attention (0.26). Crucially, though, the majority of comparisons yielded no significant results. The authors also judged the data insufficient to show that cognitive improvements attributable to exercise were caused by the fitness gains themselves, although the temporal association suggests this might be the case.
Where it fits
This review supports the broad idea that aerobic exercise benefits cognition in healthy older adults, but it complicates the popular framing that fitness gains are the engine of that benefit — the evidence simply cannot demonstrate that link yet. It leaves open whether the aerobic component is necessary at all, or whether any type of physical exercise would achieve the same result. The authors also note that some cognitive functions appear to respond to exercise while others seem insensitive to it, an unexplained pattern. They call for larger trials and for the field to agree on a smaller, standardised battery of cognitive tests to make results more reproducible.
What it means for you
For anyone past their mid-fifties, this is a reason to think aerobic exercise offers more than heart and lung benefits — the trials reviewed here show cognitive measures such as motor function, attention and processing speed moving in the right direction alongside fitness. It is equally a reason to hold the claim lightly: many comparisons showed nothing, and the causal chain from fitness to cognition is not yet established. The honest reading is that regular aerobic activity in later life is associated with sharper performance on some cognitive tests, while science works out which functions respond and why.
