Skincare after laser edged out wrinkles in split-face test

In a split-face randomised trial in 35 patients, adding an integrated topical skincare regimen after picosecond laser sessions produced greater wrinkle and texture improvement than the control routine on the opposite cheek. The laser itself significantly improved pores, brown spots and skin moisture.

Why it matters

The 755-nm picosecond alexandrite laser with a diffractive lens array is an established treatment for photoaging in Asian patients, but it works partly by temporarily disrupting the skin barrier, which can bring transient adverse effects. That raises a practical question for clinics: does what you put on the skin afterwards change either the results or the tolerability of the procedure? Post-procedure topical regimens are widely recommended and widely sold, yet controlled comparisons within the same patient are relatively rare. A split-face design offers a way to test the add-on directly while holding everything about the individual constant.

What they did

This was a single-centre, randomised, evaluator-blinded, split-face controlled study enrolling 35 patients aged 25 to 63 with facial photoaging. Each patient underwent three laser treatment sessions at four-week intervals. On one cheek — the control side — patients applied a Phyto Corrective serum plus sunscreen; on the other, they added an integrated regimen combining a CE Ferulic serum, a Discoloration Defense product containing 3% tranexamic acid, and Triple Lipid Restore 2:4:2. Efficacy was assessed with the VISIA Complexion Analysis System, the FocuSkin Skin Analyzer and a participant satisfaction questionnaire, while self-reported redness or swelling was captured by telephone follow-up.

What they found

The laser treatment itself significantly improved pores (p=0.0011), brown spots (p=0.0003) and skin moisture (p=0.0001) across both sides. The adjunctive skincare added further gains specifically in wrinkle reduction (adjusted difference -2.33, p=0.018) and texture improvement (adjusted difference -0.63, p=0.027). Subgroup patterns emerged: females over 46 showed greater wrinkle reduction (p=0.0437), while males showed greater elasticity improvement (p=0.0003). On safety, temporary erythema occurred in 13 of the 35 patients and 18 reported no adverse effects at all. Patient satisfaction scores were higher on the side receiving the integrated regimen.

Where it fits

The split-face design is a genuine strength — each participant serves as their own control, removing between-person variation in skin type, sun exposure and ageing. But the trial is small, single-centre and confined to an Asian population, and the incremental benefits were limited to two of the measured domains rather than across the board. Two of the authors are affiliated with the company that makes the tested products, which is a relevant consideration when interpreting a positive result on a branded regimen. Whether the same additive effect would appear with other post-laser moisturisers or antioxidant serums, or persist beyond three sessions, is untested here.

What it means for you

This is a reason to think the topical routine used alongside a laser course is not purely cosmetic theatre — in this trial it made a measurable, if modest, difference to wrinkles and texture on the treated cheek. It does not establish that these particular branded products are superior to alternatives, since the comparison was against one specific control routine. The laser accounted for the larger changes in pores, pigmentation and hydration; the skincare added at the margins. With 35 patients at a single clinic and industry authorship involved, replication elsewhere would strengthen the case considerably.
