Why it matters

With obesity prevalence still climbing, there is real demand for interventions that are both effective and scalable — and smartphone-era tools promise exactly that. GPS-enabled technologies can track where and how much people move, making them a candidate platform for behaviour-change programmes. What has been unclear is whether pairing these activity interventions with dietary components delivers better obesity outcomes than the activity intervention on its own. This review set out to compare GPS-enabled physical activity interventions with and without dietary strategies on obesity-related outcomes in adults.

What they did

Following PRISMA guidelines, the authors searched five databases for randomised controlled trials published between January 2000 and April 2025. Eligible studies used GPS-enabled physical activity interventions in adults aged 18 or over and reported at least one primary obesity-related outcome. Nine trials involving 1363 participants (424 males and 939 females, with mean ages ranging from 34.5 to 64.8) were included. Meta-analyses used random- or fixed-effects models depending on heterogeneity, with subgroup analyses to explore what modified the effects.

What they found

GPS-enabled activity interventions significantly reduced body weight (Hedges' g = -0.241) and body fat percentage (Hedges' g = -0.412), though the body-fat analyses showed substantial heterogeneity between studies. The subgroup comparison produced the unexpected result: activity-only interventions showed a moderate effect on weight (-0.328), while interventions that combined activity with dietary strategies showed a slightly smaller, though still significant, effect (-0.208). Interventions lasting three months or less demonstrated greater weight effects, and sensitivity and bias assessments supported the robustness of those short-term outcomes.

Where it fits

The headline that GPS-guided activity helps with weight fits comfortably within the digital-health literature, but the diet finding complicates a common assumption — that layering dietary strategies onto an activity programme should always improve results. In this pooled sample, it did not consistently do so. Whether that reflects diluted focus, differences in how dietary components were delivered, or the particular trials included remains an open question. The concentration of benefit in short-term interventions also leaves durability unresolved: the review cannot say whether these effects persist once programmes end.

What it means for you

If you use location-aware apps or wearables to structure your activity, this is evidence the approach can move the needle on weight and body fat, at least over a few months. It is not evidence that dietary change is pointless — energy intake still governs weight — but it does suggest that within these technology-led programmes, the activity component carried the effect. The small effect sizes and short-term focus are worth keeping in mind before expecting dramatic results from any app-based plan.
