Hard runs stir airway markers, not hs-CRP

Twenty healthy young men ran for 20 minutes at either 85-90% or 65-70% of maximum heart rate; the harder session raised the airway-lining marker CC16 at one hour and shifted the CC16/SP-D ratio at 24 hours, while surfactant protein D and hs-CRP inflammation did not differ between intensities.

Why it matters

The airway lining is a thin barrier that has to cope with large volumes of air moving quickly during hard exercise, and blood markers exist that are thought to reflect its integrity. Club cell protein 16 leaks into the circulation from the airway epithelium, while surfactant protein D reflects a different aspect of lung secretion; the ratio between them has been proposed as a more informative index than either alone. Whether the intensity of a single exercise bout influences these markers had not been established. Nor was it clear whether any such change is accompanied by a systemic inflammatory response, which would suggest a broader effect than a local one.

What they did

Twenty healthy young men were randomly assigned to a high-intensity group of 10 or a moderate-intensity group of 10. Both groups completed a single 20-minute bout of continuous treadmill exercise, the high-intensity group at 85-90% of maximum heart rate and the moderate group at 65-70%. Serum was collected before exercise and at one hour and 24 hours afterwards. Club cell protein 16, surfactant protein D and high-sensitivity C-reactive protein were measured by enzyme-linked immunosorbent assay, and the CC16 to SP-D ratio was calculated. Results were analysed with a two-way repeated measures analysis of variance followed by post hoc comparisons.

What they found

All four measures — CC16, SP-D, the CC16/SP-D ratio and hs-CRP — changed significantly over time in the sample as a whole. Time-by-group interaction effects, meaning the response differed by exercise intensity, were found for CC16 and the CC16/SP-D ratio, but not for SP-D or hs-CRP. Post hoc testing showed CC16 concentrations at one hour and the CC16/SP-D ratio at 24 hours were higher in the high-intensity group than the moderate-intensity group. In other words, intensity mattered for the epithelial integrity markers specifically, while the systemic inflammatory marker and surfactant protein D moved similarly regardless of how hard the men had worked.

What it actually shows

Small parallel-group randomised trial in 20 healthy young men, 10 per group, with a single 20-minute treadmill bout and blood markers only — no symptoms, lung function or long-term outcomes, and it needs replication in women and other populations.

Where it fits

The authors present this as the first demonstration that exercise intensity affects CC16 and the CC16/SP-D ratio, which extends interest in these markers from occupational and environmental exposures into acute exercise physiology. That hs-CRP did not track intensity argues against interpreting the CC16 change as a general systemic inflammatory response and points instead to something more local to the airway. Crucially, a rise in a leakage marker is not by itself evidence of injury or of any consequence for health or performance, and this design cannot tell the two apart. Open questions include what these shifts mean functionally, whether they occur in women, older adults or people with asthma, and what happens with repeated training.

What it means for you

This is a reason to think that harder efforts place a measurably different load on the airway lining than moderate ones, detectable in blood within an hour and still visible in the marker ratio a day later. It is not evidence that high-intensity exercise damages your lungs; the study measured molecules, not symptoms, breathing capacity or any clinical outcome, and healthy young men tolerated the session. The absence of an intensity effect on high-sensitivity C-reactive protein is reassuring in that the harder bout did not provoke a greater systemic inflammatory signal. With 10 men per group and one exercise session, it is an early observation to watch rather than something to change training around.
