Laser acupuncture beat sham for chronic low back pain
In a double-blind, sham-controlled trial of 60 adults with chronic non-specific low back pain, both high- and low-intensity laser acupuncture added to exercise outperformed sham laser plus exercise on pain, trunk strength, mobility, fatigue and disability. The high-intensity version delivered greater gains in trunk extensor strength and pain relief.
Why it matters
Chronic non-specific low back pain is a stubbornly common musculoskeletal complaint, characterised by persistent discomfort, weak trunk muscles, exhaustion and disability. Laser acupuncture — delivering laser light to traditional acupuncture points instead of needles — is an increasingly available add-on to exercise therapy, but two questions have remained unresolved: does it beat a convincing sham, and does the intensity of the laser matter? This double-blinded trial was designed to answer both, comparing high-intensity and low-intensity laser acupuncture head to head against a sham procedure, all delivered alongside the same exercise programme.
What they did
Sixty participants with chronic non-specific low back pain were randomised to three groups: high-intensity laser acupuncture plus exercise, low-intensity laser acupuncture over the same 15 acupuncture points plus exercise, or sham laser plus exercise. Interventions ran three times per week for four consecutive weeks. Outcomes were trunk extensor strength by isokinetic dynamometry, fatigue, pain on a visual analogue scale, lumbar flexion range of motion via the modified Schober test, and disability on the Oswestry Disability Index, all measured at baseline, after 4 weeks and at a 1-month follow-up.
What they found
Both active laser groups improved substantially more than the sham group across all measured outcomes, both immediately after treatment and over the follow-up period. The high-intensity group outperformed the low-intensity group on two measures: trunk extensor strength and pain reduction. Fatigue and disability improved similarly in the two active groups, with no difference between them. All groups showed only minor changes between the post-treatment assessment and follow-up, suggesting the gains established during the treatment window were largely maintained over that short period.

One RCT, n=60 split across three groups; a four-week intervention with only a one-month follow-up, and the authors say optimal parameters and long-term effects need further research.

Where it fits
Because every group exercised and the comparison arm received a sham laser, this design speaks directly to the common criticism that laser and acupuncture benefits are placebo effects — here, the active treatments still came out ahead. It also hints at a dose-response element, with the higher-intensity laser delivering greater strength and pain gains. Still, this is a single trial of 60 people with only a one-month follow-up, and the authors themselves note that ideal treatment parameters and long-term effects require further research before firm conclusions can be drawn.
What it means for you
For anyone managing long-standing, non-specific back pain, this trial is a reason to think laser acupuncture added to exercise may offer short-term gains in pain, strength, mobility and disability beyond exercise alone. It is not evidence that lasers replace exercise — every participant in the study exercised, and active rehabilitation remains the core of back pain care. Given the small sample and brief follow-up, treat this as an early, encouraging signal rather than settled science.
