Selenium or zeaxanthin eggs: glucose fell either way

Adults with type 2 diabetes who ate either selenium-enriched or zeaxanthin-enriched eggs twice daily for 12 weeks saw significant falls in fasting blood glucose, HOMA-IR insulin resistance and the triglyceride-glucose index, with no difference in HbA1c between the two egg types. Only the zeaxanthin group showed a significant drop in triglycerides.

Why it matters

Eggs are a cheap, widely eaten protein source, and there is commercial and research interest in fortifying them with micronutrients such as selenium or the carotenoid zeaxanthin by altering what hens are fed. For people with type 2 diabetes, the question is whether the enrichment translates into anything measurable in glucose or lipid metabolism, or whether an enriched egg is simply an egg with a marketing label. Existing debate about eggs in diabetes has mostly concerned cardiovascular risk rather than head-to-head comparisons of fortified varieties. This trial set out to compare the two enriched eggs directly on glycaemic and lipid outcomes.

What they did

Fifty-eight patients with type 2 diabetes were recruited and randomly divided into a selenium-enriched egg group of 27 subjects and a zeaxanthin-enriched egg group of 31 subjects. Each group consumed their assigned eggs twice a day for a total of 12 weeks. Fasting blood samples were taken before and after the intervention to measure blood glucose and blood lipid levels, from which the researchers derived the homeostatic model assessment of insulin resistance, the homeostatic model assessment of beta-cell function and the triglyceride-glucose index. Analyses also looked at whether baseline glycosylated haemoglobin A1c modified the response, splitting at an HbA1c of 8.9%.

What they found

After 12 weeks, fasting blood glucose, HOMA-IR and the triglyceride-glucose index fell significantly in both the selenium and zeaxanthin groups (p less than 0.05), and the authors describe the two egg types as having comparable effects on glucose metabolism. Participants who started with an HbA1c below 8.9% showed greater improvement in glucose and lipid measures than those starting higher. The zeaxanthin group additionally showed a significant fall in triglycerides (p = 0.0007) and an increase in HOMA beta-cell function that fell just short of significance (p = 0.06). There was no significant difference in HbA1c between the two groups after the intervention.

What it actually shows

Twelve-week trial in 58 adults with type 2 diabetes comparing two enriched eggs against each other, with 27 and 31 per group and no plain-egg or no-egg control — so improvements in both arms cannot be attributed to the enrichment, or to eggs at all, rather than to trial participation or diet change.

Where it fits

The study adds a direct comparison to a literature dominated by single-nutrient supplementation studies, and its most quotable result is a null one: two differently enriched eggs performed much the same on glucose measures. The triglyceride signal for zeaxanthin is a lead rather than a conclusion, resting on a single secondary outcome in a modest sample. Crucially, because both arms received eggs, the design cannot say whether the improvements reflect the enrichment, the eggs, or non-specific effects of joining a supervised nutrition trial. Longer trials with a standard-egg comparator and HbA1c as the primary endpoint would settle far more.

What it means for you

This is a reason to treat 'enriched' claims on eggs with measured interest rather than enthusiasm: within this trial, the choice between selenium and zeaxanthin made no meaningful difference to glucose outcomes. The finding that people with lower baseline HbA1c improved more is a reminder that dietary tweaks tend to move the needle most when metabolic control is not already badly disrupted. Anyone with type 2 diabetes reading this should note the trial ran for 12 weeks in 58 people and did not test eggs against no eggs, so it says nothing about whether adding eggs to your diet improves your own markers. It is a comparison of two products, not a verdict on eggs.
