VR games beat standard rehab after rotator cuff repair
A randomised trial found that adding game-based virtual reality to rehabilitation after arthroscopic rotator cuff repair produced greater improvements in pain, shoulder function, flexion range and abduction and external rotation strength than conventional therapy alone.
Why it matters
Recovery from arthroscopic rotator cuff repair is slow and often leaves the shoulder stiff and weak for months. The rehabilitation that drives recovery depends on patients doing repetitive, unexciting exercises consistently, and adherence is frequently poor. Game-based virtual reality has been proposed as a way to make that work engaging enough to sustain, turning range-of-motion drills into play. What they did
Patients who had undergone arthroscopic rotator cuff repair were randomised into two groups: one completed a game-based virtual reality programme, the other conventional rehabilitation. Outcomes were assessed at baseline, at 6 weeks and at 12 weeks after surgery. The measures covered upper-limb disability using the Disabilities of the Arm, Shoulder and Hand score, pain severity on the Numerical Pain Rating Scale, range of motion, and isometric muscle strength testing. Both within-group change over time and between-group differences at each follow-up point were analysed.
What they found
Both groups improved significantly over time in pain, function, range of motion and isometric strength at all time points. Between groups, the virtual reality arm showed greater gains in pain, function, flexion range, and abduction and external rotation strength at both follow-up points. Abduction range separated between groups only at 12 weeks. External rotation range was the clear null: no significant difference between groups at either assessment, meaning the added benefit was not uniform across every movement measured.

Whether the advantage comes from engagement, from more total repetitions, or from attention effects remains untested.
