Two skin RF devices, same measured results
In a split-face trial in 20 people, two fractional microneedle radiofrequency devices produced statistically indistinguishable skin outcomes — dermal density, hydration, sebum, pigment and redness — although one device was faster and less painful while causing more bleeding.
Why it matters
Fractional microneedle radiofrequency passes energy through fine needles into the dermis and is widely marketed for both photoageing and pitted, atrophic acne scars. The general approach already has a reasonable reputation for being effective and safe, but devices differ in needle design and how energy is delivered, and clinics frequently imply that the specific machine is what determines the result. Patients weighing up a course have no easy way to judge whether the brand on the handpiece matters. This study set out to compare two commercially available systems directly, on the same faces, using objective skin measurements rather than before-and-after impressions.
What they did
Each participant's face was split along the midline and randomly allocated so that one side was treated with the Peninsula Timerevert Needle and the other with the EndyMed. Twenty participants completed the entire course of treatment. The investigators measured melanin index, erythema index, stratum corneum hydration, sebum and dermal density at facial sites including the forehead, cheek and chin. They also recorded treatment duration, participant satisfaction, the physician's global assessment score, pain, bleeding, erythema score, and the duration of erythema and oedema, along with any other side effects, across repeated follow-up visits.
What they found
Satisfaction scores and physician global assessment scores rose progressively over time, and dermal density increased gradually after treatment, reaching a statistically significantly higher value at three months than at the other follow-up points. The head-to-head comparison, however, was largely null: there were no statistically significant differences between the two sides in satisfaction, physician assessment, erythema score, erythema duration, oedema duration, melanin index, erythema index, hydration, sebum or dermal density at the forehead, cheek or chin at any follow-up. The differences that did emerge were procedural — the treatment side took less time and hurt less, but bled more.

Where it fits
The results are consistent with the existing view that fractional microneedle radiofrequency is effective and safe for photoageing and atrophic acne scarring, and they extend it by suggesting that, between these two systems, device choice shows up in the treatment experience rather than in the measurable skin outcome. Because both sides were actively treated, the study cannot quantify how much of the improvement over time would have occurred anyway. Only two devices, one treatment course and a short follow-up window were examined, leaving open whether differences appear with longer follow-up, more sessions or other machines.
What it means for you
If you are considering this class of treatment, this is a reason to be sceptical of claims that one particular radiofrequency microneedling device delivers visibly better skin than another. What did differ here was comfort, session length and bleeding, which are legitimate things to ask a clinic about. The gradual rise in dermal density and in both patient and physician ratings over months is a reminder that changes of this kind accumulate slowly rather than appearing immediately.
