Exercise did not shift lactate in survivors

A pooled analysis of three controlled trials found no significant effect of exercise on circulating lactate concentrations in breast cancer survivors, with a weighted mean difference of 0.03 mmol/L.

Why it matters

Lactate was long dismissed as a waste product of glycolysis but is now treated as a signalling molecule involved in metabolic regulation and adaptation to training. Exercise is strongly recommended for breast cancer survivors because of gains in physical function, metabolic health and quality of life, yet how their lactate metabolism responds to exercise has barely been characterised. If survivors handled lactate differently, that could carry implications for how training is prescribed and monitored in this group. This review set out to gather and appraise whatever controlled evidence exists on exercise-induced circulating lactate in breast cancer survivorship.

What they did

The authors conducted a systematic review and exploratory meta-analysis following PRISMA guidelines, prospectively registered in PROSPERO. They searched PubMed, Scopus and Web of Science for controlled trials measuring exercise-induced changes in circulating lactate concentrations in breast cancer survivors. A random-effects meta-analysis was used to pool mean differences. From 173 screened records, four studies met the eligibility criteria for qualitative synthesis and three provided data suitable for the quantitative pooled analysis. The authors also documented how much the included protocols differed in exercise type, intervention duration and the timing of lactate sampling.

What they found

The pooled result showed no statistically significant effect of exercise on circulating lactate concentrations: a weighted mean difference of 0.03 mmol/L, with a confidence interval spanning -0.24 to 0.31. Heterogeneity was low at I² = 31.1%, meaning the individual studies broadly agreed with one another despite their design differences. Beyond that null primary finding, the review documented considerable variation across exercise protocols, intervention duration and when lactate was assessed, and noted that reporting of lactate responses was inconsistent across the small literature. The authors describe the responses as modest and inconsistently reported.

Where it fits

This is essentially a map of a gap rather than a definitive answer. With only three studies in the pooled estimate, a null result is as much a statement about the thinness of the evidence as about physiology — a real but small effect could easily be undetectable at this sample size. It does temper any assumption that lactate is a useful, reliably shifted metabolic marker in this population. The authors argue the field needs standardised exercise interventions and consistent metabolic outcome assessment, including agreed sampling timings, before firmer conclusions are possible.

What it means for you

If you follow lactate as a training metric, this is a reason to be cautious about reading it as a marker of exercise response in breast cancer survivors — the pooled evidence currently shows no clear shift, and it rests on very few studies. It does not undercut the broader case for exercise in survivorship, which the authors note rests on physical function, metabolic health and quality of life rather than on lactate. Think of it as a signal that this particular measurement has not yet earned a place in monitoring, not as a finding about whether training helps.
