Global metabolic syndrome hits up to a third of adults
Pooling 684 observational studies covering 44,979,527 adults, global metabolic syndrome prevalence was estimated at between 19% and 31%, with the highest figures reported in South Asia and in Latin America and the Caribbean. Prevalence did not differ significantly between pre-pandemic and post-pandemic studies.
Why it matters
Metabolic syndrome is the clustering of raised blood pressure, disturbed glucose and lipid handling, and central adiposity that flags elevated cardiovascular and diabetes risk. Knowing how common it is shapes screening priorities and where prevention money should go. The problem is that widely quoted global figures mostly rest on studies run before the COVID-19 pandemic, a period that changed diets, activity patterns and access to care. They also tend to treat prevalence as a purely biological number, without asking how income inequality or health system coverage might track with it. This review set out to refresh both the estimate and the context around it.
What they did
The authors searched six databases from inception to 27 October 2025 for observational studies reporting crude metabolic syndrome prevalence in adults. Two reviewers independently screened records and extracted data, and random-effects proportional meta-analyses were used to pool prevalence figures. A total of 684 unique observational studies involving 44,979,527 participants entered the quantitative synthesis, making this an unusually large pooling exercise. Meta-regression was then used to test whether socioeconomic and health system characteristics were associated with prevalence, and subgroup analyses split studies by World Bank region and by whether they were conducted before or after the pandemic.
What they found
Depending on the analysis, pooled global prevalence ranged from 19% (95% CI 15%-24%) to 31% (95% CI 30%-33%), with substantial heterogeneity throughout. Regionally, studies from South Asia reported a range of 30%-35% and those from Latin America and the Caribbean 33%-55%, both higher than other regions. Higher prevalence was significantly associated with older age, a greater proportion of female participants, greater income inequality and higher Universal Health Coverage. Notably, the pre- versus post-pandemic subgroup comparison showed no statistically significant overall difference, so the pandemic did not visibly shift the headline estimate.

Where it fits
The finding confirms the long-standing picture that metabolic syndrome affects a large minority of adults worldwide, while extending it into the post-pandemic era with far more data than earlier syntheses. The wide 19% to 31% span and the heavy heterogeneity are as informative as the central figures: differing diagnostic criteria, age structures and sampling frames mean there is no single true global number. The association with higher Universal Health Coverage is counterintuitive and probably reflects better detection and recording rather than harm from coverage. Open questions include how much of the regional spread is genuine and how definitions could be harmonised.
What it means for you
The practical message is that the cluster of risk markers behind metabolic syndrome is common rather than exotional, plausibly affecting something like a fifth to a third of adults depending on how it is counted and where. If you have several borderline results across blood pressure, waist measurement, triglycerides, HDL and fasting glucose, that combination is worth understanding as a pattern rather than as isolated numbers. This is a reason to take the clustering seriously and to know your own figures. It is also a reminder that prevalence reflects social circumstances, not just individual behaviour, since inequality tracked with higher rates.
