Med diet shifted the metabolism of ageing brains

Mild cognitive impairment sits between normal ageing and dementia, and there is intense interest in whether diet can slow the slide from one to the other. Trials of dietary patterns have hinted at cognitive benefits, but the biological route from what is on the plate to what happens in the brain is poorly mapped. Metabolomics — measuring hundreds of small molecules circulating in blood — offers a way to see which fuel-handling pathways a diet actually shifts. This study set out to ask which plasma metabolites change under a Mediterranean diet compared with a low-fat diet, and whether those changes travel alongside cognitive test scores.

What they did

The researchers report interim findings from a randomised controlled trial of a 24-week dietary intervention in 47 older adults aged 60 and over with mild cognitive impairment, of whom 42.6% were women. Participants were allocated to a Mediterranean diet, a Mediterranean diet plus probiotics, or a low-fat diet following World Health Organization recommendations. Plasma metabolites were quantified using a targeted LC-MS/MS assay, giving concentrations rather than relative signals. Cognition was assessed with ADAS-Cog-11 alongside a full neuropsychological test battery, and multivariate statistics were used to relate diet-driven metabolome changes to the cognitive scores.

What they found

The Mediterranean diet increased acetoacetic acid, a ketone body, along with several metabolites linked to the tricarboxylic acid cycle, and decreased branched-chain amino acids, with isoleucine standing out. The low-fat diet did not produce the same ketone and TCA-intermediate pattern. Separately, rises in alpha-ketoglutarate, alpha-ketoisovalerate and 2-oxocaproate, and falls in beta-alanine, L-carnitine and alpha-aminobutyric acid, were associated with improved cognitive scores. The authors interpret the overall pattern as greater metabolic flexibility in the Mediterranean diet group. The abstract reports metabolite-cognition associations rather than a definitive head-to-head cognitive outcome between the diets.

It complicates the simple assumption that any prudent low-fat diet would do the same job, because that arm did not show the ketone shift. Open questions remain: whether these metabolite changes actually cause better cognition, whether they persist beyond 24 weeks, and what the probiotic arm contributed.

What it means for you
