Saffron improved sleep and fatigue in a lung condition
In a randomised trial of 76 adults with pulmonary sarcoidosis and poor sleep, 60 days of saffron supplementation added to sleep-hygiene counselling improved sleep quality, daytime sleepiness, fatigue and quality of life compared with counselling alone.
Why it matters
Pulmonary sarcoidosis is an inflammatory lung condition in which sleep disturbances are common, worsening fatigue and reducing quality of life — and standard anti-inflammatory treatments provide limited benefit for these problems. Saffron, the spice derived from Crocus sativus, has documented anti-inflammatory and neuromodulatory properties and has been proposed as a sleep aid, but it had never been studied in this patient population. The researchers asked whether adding saffron to standard sleep-hygiene counselling could improve sleep, daytime sleepiness, fatigue and quality of life over two months.
What they did
Seventy-six adults with confirmed pulmonary sarcoidosis and Pittsburgh Sleep Quality Index scores of 5 or higher were enrolled in a single-blind randomised controlled trial. Using permuted block randomisation with concealed allocation, participants received either saffron at 30 mg twice daily plus sleep-hygiene counselling, or counselling alone, for 60 days. Outcomes were assessed at baseline and day 60 using six validated instruments: the PSQI, the General Sleep Disturbance Scale, the Epworth Sleepiness Scale, the Fatigue Assessment Scale, PROMIS fatigue measures and the SF-12 health survey. Seventy-two participants completed the study, 36 per group.
What they found
Compared with counselling alone, the saffron group improved significantly on sleep quality (adjusted mean difference on the PSQI of -2.21), general sleep disturbance (-14.60), daytime sleepiness (-2.46) and fatigue (-4.58), with all these differences reaching P < 0.001. The PROMIS global physical health score improved by an adjusted 1.65 points, and both the physical (P = 0.03) and mental (P = 0.003) components of the SF-12 quality-of-life measure also improved. No serious adverse events occurred during the trial.

Where it fits
This is the first study of saffron for sleep disturbances in pulmonary sarcoidosis, and the consistency of improvement across multiple validated scales is notable. But the design has a significant weakness: the control group received no placebo, and participants knew whether they were taking a supplement, so part of the benefit could reflect expectation rather than the compound itself. The single-blind format and single condition studied also limit generalisability. The authors explicitly state that larger, placebo-controlled studies are needed before these findings can be considered confirmed.
