Probiotics and depression: mostly null results

Pooling 27 randomised trials, probiotics showed no significant effect on depressive symptoms over short (under 6 weeks) or medium (6-12 weeks) durations, with a significant benefit appearing only in a long-duration subgroup of just 2 studies. Prebiotics and postbiotics showed no clear effects.

Why it matters

Depression is a global health problem, and one of the more appealing recent ideas is that it can be influenced through the gut. Disturbances in the gut microbiota have been linked to brain function via the gut-brain axis, which has made supplements marketed as probiotics, prebiotics and postbiotics a popular target for mood research. If swallowing a defined bacterial or fibre-based product genuinely shifted depressive symptoms, it would be a low-cost addition to existing care. The question this review set out to answer is whether the accumulated randomised evidence actually supports that promise, and whether it depends on which type of biotic is used or how long it is taken.

What they did

The authors ran a systematic review and meta-analysis, searching five databases for randomised controlled trials published between 2016 and 2024. To be included, a trial had to have a control group and to measure depression using a standard depression rating scale. Twenty-seven RCTs met those criteria. Two reviewers independently extracted the data and assessed risk of bias, and the certainty of the evidence was appraised with the GRADE framework. Results were pooled as standardised mean differences, with subgroup analyses split by intervention length: short (under 6 weeks), medium (6-12 weeks) and long (over 12 weeks). Sensitivity analyses were run to test how robust the pooled estimates were.

What they found

For probiotics, there was no significant effect on depressive symptoms in either the short-duration subgroup (6 studies) or the medium-duration subgroup (12 studies), and both of those pools showed substantial heterogeneity between trials. A significant effect did appear in the long-duration subgroup, but that estimate came from only 2 studies, albeit with no observed heterogeneity between them. Prebiotic interventions showed no significant effects at all. Postbiotics likewise demonstrated no clear effects across any duration, with variable heterogeneity. Sensitivity analyses were generally consistent with the main results, and overall certainty of evidence was rated moderate, downgraded because of inconsistency across studies.

Where it fits

This complicates the popular narrative that microbiome-targeted supplements are an established route to better mood. Rather than confirming a broad antidepressant effect, the pooled data are largely null, and the one positive signal is confined to the longest interventions and the thinnest slice of the evidence base. That pattern is interesting because it hints that duration might matter, but two trials cannot settle whether the effect is real or a chance finding in a small pool. The substantial heterogeneity across the short and medium subgroups also suggests the trials differ in strains, doses, populations and scales in ways that pooling cannot smooth over. The authors themselves call for further well-designed RCTs.

What it means for you

The honest reading is that biotic supplements are not a demonstrated fix for depressive symptoms on current randomised evidence. If there is a genuine effect from probiotics, this analysis suggests it is more likely to emerge over longer periods than over a few weeks, but that possibility rests on a very small number of trials and should be treated as a hypothesis rather than a result. Prebiotics and postbiotics have even less to stand on here. It is a reason to be sceptical of confident marketing claims linking gut supplements to mood, and a reason to keep depression care in the hands of established treatment rather than a supplement shelf.
