Probiotic cut 1.20 kg — but not BMI or cholesterol
Pooling 17 randomised trials in non-diabetic overweight or obese adults, Lactobacillus plantarum supplementation modestly reduced body weight (-1.20 kg) and waist circumference (-1.36 cm) but did not significantly change BMI, blood lipids, fasting glucose, insulin, inflammatory markers or liver enzymes.
Why it matters
Probiotics are widely marketed for weight management, and Lactobacillus plantarum in particular has been studied for effects on obesity-related outcomes. The trial literature, however, has been inconsistent: individual studies are small, use different strains and durations, and report a scattergun of outcomes from body weight to liver enzymes. That leaves consumers with confident label claims and researchers with an unresolved question. Pooling the randomised evidence and grading its certainty is the only way to see whether a real, if modest, effect exists — and, just as importantly, which of the many advertised benefits the data do not support.
What they did
The authors systematically searched PubMed, Embase, the Cochrane Library, Web of Science, Google Scholar and ClinicalTrials.gov up to May 2026 for randomised controlled trials of L. plantarum supplementation in non-diabetic overweight or obese adults. Seventeen RCTs met the criteria. Random-effects models pooled weighted mean differences with 95% confidence intervals across body composition, lipid profile, glycaemic indices, inflammatory biomarkers and liver enzymes. Subgroup analyses examined strain type (single versus multi-strain), intervention duration and sample size. Risk of bias was assessed with the Risk of Bias 2 tool and the certainty of the evidence was rated using the GRADE approach, so the review reports not just effect sizes but how much confidence they deserve.
What they found
L. plantarum significantly reduced body weight by 1.20 kg (95% CI -1.92 to -0.48; P < 0.001; I²=47.4%) and waist circumference by 1.36 cm (95% CI -2.27 to -0.45; P = 0.003; I²=58.5%). BMI did not change significantly (-0.25 kg/m²; p = 0.102) and heterogeneity for that outcome was high at I²=82.8%. There were no significant overall effects on triglycerides, total cholesterol, LDL-C, HDL-C, fasting blood sugar, insulin, IL-6, CRP, TNF-α, ALT or AST. Larger improvements in weight and waist appeared with multi-strain formulations, 12-week interventions and samples exceeding 50 participants; durations longer than 12 weeks modestly improved lipid profiles, and multi-strain products reduced fasting glucose only in the larger 12-week studies.

Where it fits
This complicates the broad marketing story around probiotics and metabolic health. It supports a small, real signal on body weight and waist circumference while contradicting the wider claim set — lipids, glycaemia, inflammation and liver enzymes all came out null overall. The mismatch between a weight change and no BMI change, with very high heterogeneity for BMI, is itself a caution that the pooled estimates are unstable. Subgroup patterns favouring multi-strain formulations and at least 12 weeks are hypothesis-generating rather than definitive, and the authors conclude that longer and larger trials are warranted.
What it means for you
If you are wondering whether a L. plantarum supplement will reshape your metabolic profile, this pooled evidence suggests not: the cholesterol, blood sugar, inflammation and liver markers did not budge. What remains is a small average change in body weight and waist circumference — the kind of difference that is measurable in a trial but easy to lose in normal week-to-week variation. It is a reason to treat probiotics as a possible marginal adjunct rather than a weight-loss strategy, and to be sceptical of labels promising broader metabolic benefits that the trials did not deliver.
