Real food, not shakes, lifted older adults' protein
A meta-analysis of whole-food dietary interventions in adults aged 50 and over found they raised daily protein intake by 12.2 g/day compared with controls, with bigger effects when programmes ran longer, used individual counselling and set an absolute protein target.
Why it matters
Getting enough dietary protein helps preserve muscle mass and strength as people age, which is why protein supplements are so widely recommended to older adults. The problem is durability: supplement-driven increases in intake often fade once the trial or the tub ends. Changing what appears on the plate — ordinary foods, meal patterns, portion habits — is the alternative route, but it is harder to deliver and its effectiveness has not been summarised systematically. This review asked how much whole-food approaches actually shift protein intake, and which design features make them work.
What they did
The authors searched PubMed, Embase and Web of Science for studies in community-dwelling adults aged 50 years or older that tested whole-food interventions lasting at least 12 weeks and measured daily protein intake in grams per day or grams per kilogram per day. Sixteen studies with 1712 participants (33% male) were included in the systematic review, and risk of bias was assessed with the revised Cochrane tool for randomised trials. Eleven of those studies, covering 1207 participants and having a control group, were pooled in a random-effects meta-analysis with protein intake as the primary outcome. Intervention characteristics were also examined to see which were associated with success.
What they found
Whole-food interventions produced a significantly greater rise in absolute protein intake than control conditions, at +12.2 g/day (95% CI +5.0 to +19.4). Relative intake also increased, by +0.13 g/kg/day, though the confidence interval reached down to 0.0, making that result far more fragile. Characteristics associated with greater effectiveness were a longer duration — more than 3 months and up to 6 months — individual counselling sessions, in-person or hybrid delivery, and setting an absolute protein target in grams per day or grams per meal. The review reported intake, not downstream changes in muscle mass or function.

Where it fits
This supports the idea that food-based strategies are a genuine option rather than a weaker substitute for supplements, and it puts a number on the shift they achieve. It does not pit whole foods directly against supplementation in a head-to-head comparison, so the claim that food changes last longer remains an inference rather than a demonstrated result here. The wide confidence interval and the borderline relative-intake finding show real variability between programmes. The bigger open question is whether an extra 12.2 g/day translates into measurable preservation of muscle, strength or independence over years.
What it means for you
If you or an older relative are trying to eat more protein, this is a reason to think that reorganising real meals can move intake substantially, not just marginally. The features linked to success were unglamorous: sustained contact over months, one-to-one guidance, face-to-face or mixed delivery, and a concrete gram target rather than vague encouragement. That said, the review measured what people ate, not what happened to their muscles, so the health payoff is assumed rather than shown. Treat it as evidence about how behaviour change works, not as a specific eating plan.
