Synbiotics nudged insulin markers in PCOS trials

A systematic review of 11 randomised trials found that at least eight weeks of probiotic or synbiotic supplementation in overweight and obese women with PCOS improved insulin resistance markers, lipid profiles and hormonal measures, including higher SHBG and lower total testosterone. Synbiotics had more pronounced effects than probiotics or prebiotics alone.

Why it matters

Polycystic ovary syndrome is a common endocrine disorder in women of reproductive age, and two of its defining features are insulin resistance and hormonal imbalance. Because the gut microbiota influences how the body handles glucose and lipids, there has been growing interest in whether feeding or supplementing the microbiome could shift those markers. Probiotics, prebiotics and synbiotics are cheap, widely available and low-risk compared with drug therapy, which makes the question commercially attractive and therefore worth scrutinising carefully. Before anyone recommends them, the actual randomised evidence needs to be laid out.

What they did

The authors searched PubMed, Cochrane CENTRAL, Scopus, Web of Science and Embase following PRISMA guidelines, looking for randomised trials of probiotic, prebiotic or synbiotic supplementation lasting at least eight weeks in women diagnosed with PCOS by the Rotterdam criteria. Eleven trials met the criteria, all conducted in Iran, in overweight or obese women aged 15 to 48. They extracted data on participants, interventions and outcomes relating to insulin resistance and hormones, including HOMA-IR, fasting glucose, insulin, LDL, triglycerides, HDL, sex hormone-binding globulin and total testosterone. The results were synthesised narratively rather than pooled statistically.

What they found

Probiotic and synbiotic supplementation was associated with significant improvements in insulin resistance, seen as reductions in HOMA-IR, fasting glucose and insulin. Lipid profiles also moved favourably, with lower LDL and triglycerides and higher HDL. On the hormonal side, sex hormone-binding globulin rose and total testosterone fell. Synbiotics — combining live organisms with the fibres that feed them — produced more pronounced effects than either probiotics or prebiotics alone. Adherence across the trials was high and reported side effects were minimal, which matters for any intervention intended to be taken daily for months.

Where it fits

This adds structure to a promising but immature literature. The direction of effect is consistent across trials, which is encouraging, but the authors themselves flag the constraints: small sample sizes, a homogeneous population and short intervention durations. Every included trial came from a single country, and all participants were overweight or obese, so it is unknown whether lean women with PCOS respond similarly. Without pooled effect sizes, the magnitude of benefit is also hard to judge against established approaches. Larger, longer, multicentre trials with standardised methods are the obvious next step.

What it means for you

This is a reason to take the microbiome seriously as a lever on insulin resistance and hormone markers in PCOS, not a reason to treat a supplement as therapy. The combination products fared best in these trials, but that comparison was indirect rather than head-to-head. Because the evidence comes from one country and one body-composition profile, a reader outside that group cannot assume the same response. Anyone managing PCOS would sensibly view this as an area to watch and to discuss with a clinician rather than a settled answer.
