133 reviews agree: exercise sharpens the mind

An umbrella review pooling 133 systematic reviews and 2,724 randomised trials found exercise improves general cognition, memory and executive function across all populations and ages. Effects were generally larger for low- and moderate-intensity activity.

Why it matters

The idea that exercise benefits the brain as well as the body has been tested in thousands of trials, but individual studies and even individual meta-analyses can disagree, differ in populations, and vary in quality. An umbrella review — a systematic review of systematic reviews — sits at the top of the evidence hierarchy and can ask the biggest version of the question: across all ages and populations, does exercise reliably improve cognition, and does the type, intensity or duration of exercise matter? That is what this study set out to establish.

What they did

The authors searched eleven databases for systematic reviews and meta-analyses of randomised controlled trials evaluating exercise effects on general cognition, memory and executive function. Data extraction and risk-of-bias scoring were conducted in duplicate, with review quality assessed using AMSTAR-2. In total, 133 systematic reviews were included, covering 2,724 randomised trials and 258,279 participants. Effect sizes were pooled using random-effects models and reported as standardised mean differences, with subgroup analyses for participant and intervention characteristics.

What they found

Exercise significantly improved general cognition (SMD 0.42), memory (SMD 0.26) and executive function (SMD 0.24) — modest but consistent effects. Improvements in memory and executive function were greater in children and adolescents than in adults and older adults, and people with attention-deficit/hyperactivity disorder showed greater executive-function gains than other populations. Effects were generally larger for low- and moderate-intensity interventions, and shorter interventions of 1-3 months and exergames — video games requiring physical movement — showed the largest effects on general cognition and memory. Findings remained statistically significant after excluding reviews rated low or critically low quality.

Where it fits

This is among the broadest syntheses yet of the exercise-cognition literature, and it consolidates a picture that individual trials could only sketch: the cognitive benefit of exercise is real, general and not confined to any one population. The finding that low- and moderate-intensity activity produced generally larger effects complicates the assumption that harder is always better for the brain. The strong showing for exergames and shorter interventions raises open questions about novelty, engagement and how long benefits persist beyond the intervention period.

What it means for you

The practical picture is unusually inclusive: benefits appeared across ages and populations, and intensity did not need to be high — even light activity was associated with cognitive gains. That makes this a reason to view movement of almost any kind as relevant to how well you think, remember and plan, not just to fitness. The effect sizes are modest rather than transformative, so this is about a reliable edge accumulated across a population of trials, not a dramatic individual change.
