The outcomes assessed spanned functionality, frailty status, muscle strength, physical fitness, cognition, quality of life and endurance, and risk of bias was appraised for each trial.

Significant improvements were observed in functionality, handgrip strength and lower-limb strength, with the largest effects in programmes that used progressive intensity and ran for longer. Combined interventions pairing band exercise with nutritional education showed a greater impact on reducing frailty levels than exercise alone. Where it fits
The findings support elastic-band training as a safe, affordable and feasible option for frail older adults, consistent with the broader case for resistance exercise in later life. What this review adds is evidence for a specifically low-cost, low-barrier format that could plausibly be rolled out in community and institutional settings. But the base is thin: four trials, a small combined sample, mostly high risk of bias, and no long-term follow-up. Whether gains hold over time, and whether autonomy can be shifted at all, remain open questions for better-designed trials.
What it means for you
If you or someone you care for is dealing with frailty in later life, this is a reason to think meaningful strength and function gains do not require a gym — progressive work with elastic bands over several months improved grip, leg strength and everyday function in these trials. The combination of exercise with nutrition education looked stronger than exercise alone. The evidence is small and methodologically weak, so confidence should be measured. Still, the direction of the finding favours accessible, low-cost resistance work as a viable tool against frailty.
