Why it matters

Eating at night, when the body is biologically primed for sleep rather than digestion, has been repeatedly associated with worse cardiometabolic outcomes in shift workers. The obvious question is whether deliberately changing when shift workers eat — pushing food outside the overnight window — actually improves measurable markers, or whether the association simply reflects everything else that comes with shift work. Observational data cannot answer that, so this review deliberately restricted itself to randomised controlled trials. It set out to establish what intervention evidence genuinely exists, and how much weight it can bear.

What they did

The authors searched MEDLINE, the Cochrane Library, Web of Science, Embase, CINAHL Complete and Scopus with a predefined strategy, including only randomised controlled trials in adults aged 18 or over working in shift environments. Four trials met the criteria. Three assigned fasting periods during the night shift, spanning windows between 19:45 and 06:30 h, while the fourth used a 10-hour self-selected time-restricted eating schedule. Outcomes across the trials included fasting insulin, HOMA-IR, HDL-C, VLDL-C particle size, body weight, and postprandial measures such as glucose area under the curve, non-esterified fatty acid area under the curve and 2-hour glucose.

What they found

The postprandial results were the clearest: 10.75 hours of night fasting produced significantly lower glucose AUC, lower non-esterified fatty acid AUC and lower 2-hour glucose than a meal-and-snack-at-night pattern. The 10-hour self-selected time-restricted eating trial reported significantly lower VLDL-C particle size versus control. Of the four trials reporting fasting insulin and HOMA-IR, only one showed a significant change, following an 8.5-hour overnight fast. Body weight was slightly lower after a short overnight fast. No trial showed a significant change in HDL-C. The authors stress that so few trials limit generalisability and raise concerns about how robust these preliminary positive outcomes are.

Where it fits

This turns a well-established observational story — night eating tracks with poorer cardiometabolic health — into a much thinner interventional one. The postprandial signals are consistent with the wider time-restricted eating literature, where after-meal glucose handling tends to respond before fasting markers or lipids do. But with four trials, differing fasting windows and short durations, nothing here is settled. The open questions are large: whether these acute metabolic shifts persist, whether they translate into hard cardiometabolic risk over months or years, and whether fasting through a night shift is sustainable alongside the fatigue and hunger of overnight work.

What it means for you

If you work nights, this is a reason to think that shifting food out of the overnight window may improve how your body handles a meal, particularly your post-meal glucose. It is not evidence that it will change your cholesterol profile, and the effect on insulin resistance appeared in only one of the four trials. Given how few trials exist and how short they were, this sits firmly in the promising-but-unproven category. It is a signal worth watching rather than a settled conclusion about shift-work nutrition.
