The authors systematically searched PubMed, EMBASE and the Cochrane Library from inception to April 2025 for cohort studies comparing mortality risks in people with atopic dermatitis against controls, following PRISMA guidelines. Two reviewers independently screened studies, extracted the data and assessed study quality using the Newcastle-Ottawa Scale. Hazard ratios were then pooled using random-effects models, and the team ran sensitivity analyses, subgroup analyses and assessments for publication bias. Nine cohort studies met the inclusion criteria and were combined in the final analysis.
What they found
People with atopic dermatitis had a significantly higher risk of all-cause mortality, with a pooled hazard ratio of 1.222 (95% CI 1.117-1.336). Cause-specific risks were also elevated: mortality from infectious diseases (HR 1.606), respiratory diseases (HR 1.381), gastrointestinal diseases (HR 1.658) and cancer (HR 1.129). Notably, the confidence intervals for the gastrointestinal and cancer estimates only just cleared the line of no effect, so those signals are less firm. Subgroup analyses showed a higher all-cause mortality risk in prospective studies than retrospective ones, and significantly higher risks in studies from Europe and Asia.

This pooled analysis strengthens the case that atopic dermatitis is associated with health risks beyond the skin, echoing a broader shift towards viewing it as a condition with systemic implications.
