What it is
Thymosin alpha-1 is a naturally occurring peptide involved in regulating the immune system. As the drug thymalfasin it is approved in several countries for specific immune-related uses — which sets it apart from most compounds marketed to gym-goers.
Composition
It is a synthetic version of a peptide fragment produced by the thymus gland. In its approved form it is a defined pharmaceutical; it is also sold as a research chemical.
Proposed mechanism
It modulates immune-cell activity. That immune role is the basis for its approved medical uses, and for the broader "wellness" and recovery claims made online.

Extrapolating from its approved uses to a fitness benefit is where the marketing overreaches; that benefit is not established.
Legal & regulatory status
Approved in a number of countries for immune-related medical uses, but not for fitness or anti-ageing. Notably, WADA has clarified that thymosin alpha-1 is not prohibited — unlike thymosin beta-4 (TB-500), which is banned. Two similarly named peptides therefore have very different anti-doping status.
